Hourly wage: To Be Determined
Responsibilities: Assist Supported Living with a variety of functions including: daily note sorting, filing and monitoring corrections, matching duplicate copies of daily notes to originals, copying and mailing, logging of incoming receipts, faxing, clerical and project support. Prepare timesheets and daily notes for review and corrections on a weekly basis. Sort all other staff paperwork sent in for the office staff. Ensure the daily notes for the persons served are sorted and alphabetized by the last name of the person served; and filed according to date and time on a weekly basis. Sort daily notes for corrections; follow up with staff members for daily note and timesheet corrections. Scan daily notes into the computer. Maintain database of Supported Living Emergency Drills. Process consumer end of month paperwork and distribute to the appropriate Supported Living office staff member. Scan consumer financial information. Review receipts that have been sent in from Community Support Staff, making sure the receipts are attached to the check log. Open and process all incoming bills and statements for persons served. Fax all out going prescriptions and requests to the Pharmacy. Provide direct care as needed and participate in the on call rotation. Assist with the completion of Medication Destruction Logs. Must be able to care for the physical needs of persons served including feeding, hygiene, and toileting. Must be capable of assisting in physically transferring/moving consumers when needed. Physical activities may include but not be limited to: lifting up 25-50 pounds (depending on person served); bending; reaching; stretching; grasping; visual acuity; walking; squatting, guiding; and transporting. Assist with the use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer Lift, Shower Chair, Blood Sugar Testing Equipment as needed. Coordinate and transport persons served to medical appointments and/or procedures as needed.
Minimum qualifications: High School Diploma required. Experience with the public preferred. Previous experience working with persons with disabilities preferred. Must possess excellent communication skills both written and oral, and strong teamwork skills. Proficiency in Microsoft Word and Excel. Must be able to perform such functions as operate a computer, and access existing records storage. Must possess or be able to obtain CPR and first aid certification annually and perform as necessary. Must possess or be able to obtain CPI certification annually and perform as necessary. Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier. Must possess and maintain reliable transportation that meets state requirements for transporting persons served.
